How to register with your local ASL and choose a family doctor
Italy runs a public health service, the Servizio Sanitario Nazionale (SSN), managed locally by your ASL — the Azienda Sanitaria Locale, called ATS or ASST in some regions. Registration is what turns a residence permit into actual access to doctors and hospitals.
There are two routes. Iscrizione obbligatoria is free and applies if you work, are self-employed, are registered as unemployed, or join a family member who qualifies. Iscrizione volontaria is a paid annual option for everyone else, such as students and holders of elective-residence permits.

Find your ASL district
Your ASL is determined by your address. Search for the distretto sanitario covering your street, not simply the nearest office.

Work out which route applies
Employment, self-employment or family reunification usually mean free mandatory registration. Study or elective residence usually mean the paid voluntary route.

Pay the fee if voluntary
For voluntary enrolment, pay the annual contribution first with a modello F24 at a bank, post office or tabaccheria, and keep the receipt.

Submit documents at the ASL

💶 Costs and What You Still Pay
Mandatory registration is free. Voluntary registration is an annual contribution that used to be a flat amount but is now calculated from income, with different minimums for students and other categories — figures start in the several-hundred-euro range and rise well beyond a thousand for higher incomes, so confirm the current bands with your ASL. Even once enrolled, you pay a ticket, a small co-payment, for specialist visits and tests unless you hold an esenzione for income or a medical condition. Visits to your medico di base are free.
